Form 956
Advice by a migration agent/exempt person of providing immigration assistance (134KB PDF file)

Notes:
The use of this form has changed.
Migration agents are required to notify the department if they are representing a visa applicant. They can give this notification by completing Form 956. Form 956 also allows those people who are exempt from registration to advise the department that they are providing immigration assistance.
The migration agent/exempt person may also be appointed as the authorised recipient on the new Form 956. If the visa applicant wants to appoint an authorised recipient who is not a migration agent/exempt person, then they will need to complete Form 956A - Appointment or withdrawal of an authorised recipient.

Form 956A
Appointment or withdrawal of Authorised Recipient (116KB PDF file)

Notes:
This is a new form.
Form 956A is to be used for the appointment of an authorised recipient in situations where Form 956 is not appropriate.
Form 956A is only to be used in limited situations where a person who is neither a migration agent nor an exempt person is being appointed by the client to be the client's authorised recipient.
